Statin initiation and all-cause mortality in incident statin-naïve dialysis patients

Background and aimsCardiovascular disease is the main cause of death in end-stage kidney (ESKD) patients. We aimed to explore association between statin initiation after starting dialysis all-cause mortality statin-naïve ESKD patients.MethodsWe analyzed nationwide claims data incident patients from 2010 2017 South Korea. Patients who had previous cardiovascular events or were administered statins before excluded. The study group included receiving within 1 year initiation. control was organized propensity-score matching with age, sex, time initiation, underlying diabetes mellitus hypertension. outcomes major events.ResultsWe 1596 started treatment 1:1 matched statin-nonusers. During 9438 person-year follow-up, 468 deaths 264 adverse (MACEs) occurred. Statin associated a reduced risk (adjusted hazard ratio (aHR) 0.72, 95% confidence interval (CI) 0.60–0.87, p = 0.001), but not MACE incidence (aHR 1.06, CI 0.83–1.36, 0.62). In particular, prescribed recommended dosage according Kidney Disease Improving Global Outcomes guidelines showed lowest 0.55, 0.40–0.75, < 0.001).ConclusionsStatin lower As indication bias may be present observational setting, further prospective studies are warranted validate cases.
